Treatment Services

Alternatives Program

Alternatives Program is a medically supervised drug free outpatient program service that began by serving the Criminal Justice community in 1990 and has expanded services to include many other populations. Between all sites the program has a census of about 1,000 clients, many of whom are referred by NYC Probation, NYS Parole, Brooklyn and Manhattan Drug Courts, US Parole, and other branches of the Criminal Justice system. Referrals from other sources and self-referrals are also accepted. Services are offered in the Bronx, Brooklyn, and Queens.

Bio-psychosocial assessments, physical examinations, vocational assessments and referrals, urine testing, life skills, HIV/AIDS and other Health Education, coping skills, relapse prevention, emotion management, conflict management, and group and individual counseling are offered to these clients on a daily basis. Clients are given a light meal and tokens to help defray the cost of their daily attendance.

Psychiatric, psychological, and social work services are available on site and by referral to Narco Freedom's Community Counseling Center. Primary medical care, intensive HIV/AIDS case management, intensive case management for women at risk of losing their children to TPR, and all of the HIV and Perinatal services of Narco Freedom are available to the patients of this program.

Alternatives has received the highest ratings possible upon inspection by regulatory agencies, (e.g., New York State Office of Alcoholism and Substance Abuse Services, U.S. Center for Substance Abuse Treatment). In addition, Alternatives was used as a model for New York City Department of Probation when they were developing group counseling capabilities for their probation officers.

A staff consisting of CASACs, Counselors in training for licensure, a Registered Nurse, a Psychiatrist, and a CSW provide services to Alternatives clients. Staff are culturally and linguistically similar to the clients served. Approximately 1,000 patients participate in treatment at the Alternatives programs..

Hope Case Management

Hope Case Management provides extensive, well coordinated case management services to HIV/AIDS. Comprehensive case management includes case management services to family members and significant others who may have an impact on the lives of the identified client. In these programs, participants receive a full, comprehensive bio-psychosocial needs assessment, an extensive service plan, and coordinated linkage to all services required to help them and their families live healthy, productive lives.

Family members are included in the target population. For example, if a client's husband needs employment, the case management team will link the husband to local employment resources and follow the case until he is employed. If there is a teenager in the home who is abusing drugs, the case management team will link the teen to substance abuse treatment and follow him/her through the treatment process.

Hope Case Management is inspected regularly by the New York State AIDS Institute. The program has received excellent ratings.

Hope Case Management uses a team approach: a case manager, a case management technician, and a community follow-up worker team up to provide intensive services to up to 40 clients and their families. Eleven teams serve the South Bronx area. Three teams serve the Brooklyn and the Staten Island facilities. In addition to case management teams, CSW Assistant Directors and Program Directors, Quality Assurance Specialists, Case Records Specialists, Clerical Support Staff and Security Professionals make up the staff of this program. Hope Case Management and Family Connections currently serves 400 clients. Services are provided by culturally and linguistically appropriate staff in English and Spanish.

Independence Program

Narco Freedom Independence Program provides services directed toward alcohol and other drugs with alcohol being the primary drug of abuse in most cases. The program offers intensive alcoholism counseling and education, family therapy to three populations - alcoholics who are self referral or referred from other area programs (including detoxification centers), alcoholic clients of Narco Freedom's Methadone programs, and individuals who struggle with problems because of the impact of the alcohol abuse of significant others in their lives.

Independence provides clients with an extensive bio-psychosocial assessment, physical examination, individual and group counseling, health and specific HIV education, vocational assessment and referral, nutritional counseling, spirituality groups, and family therapy. Relapse prevention, and twelve step groups enable clients to learn about, enter into, and sustain a recovery program.

As with all Narco Freedom programs the additional support of New Beginnings Community Counseling Center, Neighborhood and Family Community Health Center, Hope Case Management (for people living with HIV/AIDS), and the comprehensive support services of Narco Freedom are easily accessible to any Independence client.

Methadone Maintenance Treatment Program (MMTP)

Narco Freedom's Methadone programs provide complete biopsychosocial assessments, physical examinations, vocational assessment and referral, urine testing, chemotherapy, individual and group counseling, alcoholism counseling, assistance with entitlements, and case management services. Psychiatric assessments, medications and psychotherapy are available on a short-term crisis intervention basis on site, and long-term psychotherapy and psychiatric chemotherapy are available by referral to Narco Freedom's New Beginnings Community Counseling Services.

Methadone clients have easy access to Narco Freedom's on-site or nearby HIV and general primary medical services, intensive case management for HIV/AIDS patients and their families, and Directly Observed Therapies for TB. Methadone clients are enrolled in any of the social support programs offered by the organization on an as needed basis.

Our highly qualified staff of helping professionals has seen success after success. They are able to translate this knowledge into concrete examples that increase hope and offer encouragement. As patients become more hopeful their compliance with medical and drug treatment protocols, engagement in mental health services, and risk reducing behaviors position them for healthier outcomes and encourage their family members to follow their example.

Narco Freedom has consistently reached out to family members of Methadone clients to help them deal with the problems addiction has caused them. Several Narco Freedom initiatives have been developed to meet these needs. These include mental health services, primary medical care, family counseling in drug free and alcoholism out patient programs and others.

Narco Freedom Methadone Treatment facilities have received the highest possible ratings by regulatory agencies such as the New York State Office of Alcoholism and Substance Abuse Services, U.S. Drug Enforcement Agency, New York State Department of Health, U.S. Center for Substance Abuse Treatment, and all of these services received the highest marks possible during the accreditation process by the Joint Commission. These evaluations confirm that the Comprehensive Methadone Model of Narco Freedom successfully provides quality care that improves the lives of this high risk population.

Program staff includes CASACs, Social Workers, Registered Nurses, Licensed Practical Nurses, Medical Doctors (Internists and Psychiatrists), Nurse Practitioners, HIV Prevention Specialists, Certified Medical Records staff, Credentialed Security Guards, and support personnel. Staff members are culturally and linguistically similar to the clients they serve, for example, sixty-five percent of our staff in the Bronx speak Spanish. A total of 3350 MMTP and 200 KEEP patients attend these programs.

Neighborhood & Family Health Center Clinics

Patients are welcomed into a warm, caring atmosphere where little waiting is required before seeing their physician. They are able to maintain a regular provider with whom they feel comfortable and have established confidence and rapport.

A unique aspect of the primary care program is the Case Management component. This unit provides supportive counseling and linkage to entitlements, psychosocial referrals, and follow-up.

Outreach to community residents and program participants has enabled this clinic to grow rapidly. The clinic's caring and compassionate environment has enabled the program to maintain ongoing relationships with their patients.

The program has received excellent reviews from the U.S. Bureau of Primary Health Care, New York State Department of Health, New York State Immunization Campaign, NYS AIDS Institute, and other regulatory inspectors.

The staff of the Center includes a Board Certified Immunologist/ASAM approved Medical Director, Board Certified Pediatricians, Internists, Allergist, Podiatrist, Child and Adult Optometrists, Certified Nurse Practitioners, Nurse Midwife, Physician's Assistants, RNs, LPNs, Medical Assistants, Medical Records staff, a CSW Case Manager and two Case Management Technicians and clerical support staff. More than 15,000 patients are now receiving health care from Neighborhood and Family Community Health Centers. New Beginnings Community Counseling Center

The New Beginnings Community Counseling Center provides quality mental health services to the adults and children in the Mott Haven community and those patients of Narco Freedom programs who indicate need. Of particular concern to the program are Mentally Ill Chemical Abusers (MICA) who attend Narco Freedom programs and are in need of coordinated mental health and AOD services.

Available on site and through satellites are psychiatric, psychological, and social work assessment, crisis intervention, pharmacotherapy, case management, family therapy, and individual and group psychotherapy, behavior management and psychoeducational services.

Family members of adult and child patients will be able to participate in behavior management and psychoeducational groups and will be encouraged to attend a family members support group held monthly.

Children and adults who are able to manage their illness on an outpatient basis will be able to access medication as indicated, play therapy, individual counseling, group counseling, and AOD services within the Narco Freedom system.

A satellite site for mental health services especially for methadone clients is, or will soon be located in each Narco Freedom MMTP site. In these satellites, intake, assessment, medication monitoring, and crisis intervention services will be available.

A full array of chemotherapy services is available in the New Beginnings program. These include medication management, blood level monitoring, and other services where indicated. Given the focus of Narco Freedom on AOD services, doctors are trained to understand the impact of substance abuse on symptomatology and the need for taking AOD treatment concerns into account in mental health services.

Services are provided by culturally and linguistically appropriate staff in English and Spanish. Staffing of the New Beginnings Mental Health Program includes an MSW program director, full-time and part-time psychiatrist, a part-time psychologist, several MSW/CSW level therapists and LMHCs, and two mental health technicians. Over 800 patients receive services through New Beginnings Counseling Program.

Outpatient Substance Abuse Treatment

New Beginnings Community Counseling Center has been providing comprehensive outpatient mental health services and quality care to the child, adolescent and adult population of the South Bronx since 1996. The program, staffed by psychiatrists, psychologists and social workers, operates under New York State and New York City Department of Mental Health laws and regulations.

Patients are assigned to a therapist for therapy and if needed to a psychiatrist for medication management. Patients are seen by appointment; however, in a time of crisis patients will be seen without an appointment. Emphasis in treatment is on individual and group psychotherapy, symptom management and pharmacotherapy. Case management is also an integral part of the program.

Patients are usually seen on a weekly basis. The staff adopts eclectic multicultural and sensitive approaches in an attempt to treat the patient in a manner that alleviates symptoms and improves functioning. Marital therapy, family therapy and play therapy are also offered.

New Beginnings Community Counseling Center provides a variety of specialty groups for individuals who are seeking help with depression, anger management, medication management, parenting skills and other issues. Groups are offered in both English and Spanish.

Residential Alcohol & Substance Abuse Treatment

Regeneration is a residential treatment program for women and their pre-latency age children (infancy through age 9 years). Women live in the program with their children and progress through a series of four phases developed to help them learn to function more independently and overcome addiction to drugs and self-destructive behaviors. The program design requires approximately six months of intensive treatment and three months of transition to independent living to complete.

The women attending the program come with many needs which require assistance from a case manager; these concerns have to be resolved before they enter treatment or they will delay the client's ability to commit to recovery. Often crisis events happen during treatment which require intervention. Finally, participants need to be assisted with discharge planning. Three full-time case managers offer these services.
