INTENSIVE CASE MANAGEMENT
Hope Case Management provides extensive, well coordinated case management services to HIV/AIDS. Comprehensive case management includes case management services to family members and significant others who may have an impact on the lives of the identified client. In these programs, participants receive a full, comprehensive bio-psychosocial needs assessment, an extensive service plan, and coordinated linkage to all services required to help them and their families live healthy, productive lives.
Family members are included in the target population. For example, if a client's husband needs employment, the case management team will link the husband to local employment resources and follow the case until he is employed. If there is a teenager in the home who is abusing drugs, the case management team will link the teen to substance abuse treatment and follow him/her through the treatment process.
Hope Case Management is inspected regularly by the New York State AIDS Institute. The program has received excellent ratings.
Hope Case Management uses a team approach: a case manager, a case management technician, and a community follow-up worker team up to provide intensive services to up to 40 clients and their families. Eleven teams serve the South Bronx area. Three teams serve the Brooklyn and the Staten Island facilities. In addition to case management teams, CSW Assistant Directors and Program Directors, Quality Assurance Specialists, Case Records Specialists, Clerical Support Staff and Security Professionals make up the staff of this program. Hope Case Management and Family Connections currently serves 400 clients. Services are provided by culturally and linguistically appropriate staff in English and Spanish.

Since its inception, Narco Freedom and its affiliates have cared for thousands of addicts and their families; the current census is over 6,500 patients in treatment for alcohol and other drug addiction, and more than 30,000 patients, family members and community residents served through medical, mental health, case management, and other support programs.
Narco Freedom has extensive experience dealing with government agencies around licensing, rate setting, and compliance with regulations. The program is licensed by the New York State Office of Alcoholism and Substance Abuse Services and the Department of Health as an Article 28 facility; Article 31 license regulates mental health. The agency meets the requirements for licensing by the United States Drug Enforcement Agency, Substance Abuse and Mental Health Services Administration and the Joint Commission on the Accreditation of Health Care Organizations.
Narco Freedom supports a full complement of service offerings to meet the needs of program participants. For example, Narco Freedom sponsors five methadone maintenance programs and three methadone to abstinence programs across the City. The Narco Freedom Methadone Maintenance Treatment component employs the "wrap around" model of care that increases continuity of care and protects vulnerable clients from "falling through the cracks".
